Your notice stays on your device unless you choose an AI rewrite.

When you open a PDF or photo, the site’s document-reading code runs in your browser. It extracts text locally, then a small rules-based parser looks for likely fields such as insurer, plan type, service, reason, date, deadline, and reference number. The selected file and extracted text are not sent to our application server or an AI service. You must verify every field against the original notice because text recognition can misread dates, deadlines, names, and reference numbers.

Text-based PDFs are supported for up to 12 pages. Photos use on-device optical character recognition. A scanned PDF may need to be opened as a clear photo instead. Files larger than 10 MB are not accepted.

The local draft and form values remain in the current browser page. Copying or downloading the draft is an action you initiate on your device. The current builder does not create an account or save a case.

If you explicitly select the AI rewrite and consent, the site sends the confirmed insurer, coverage type, denial type, service, stated reason, dates, reference number, urgency choice, and the explanation you wrote to the configured AI provider. The original PDF, photo, and complete extracted notice text are not sent. The response is used to produce the rewrite and is not intentionally placed in analytics events or URLs. AI providers and hosting infrastructure may still process request data under their applicable service terms.

Do not send denial letters, medical records, member IDs, Social Security numbers, or other sensitive health information by ordinary email. The public correction and press contact is not a secure channel for personal health information.

Like most websites, the hosting infrastructure may process standard request information such as IP address, browser type, requested page, and time of access for security and operations. The application does not intentionally add appeal fields to URLs or analytics events. If you choose the AI rewrite, the confirmed fields are transmitted in the body of that one server request.

Browser extensions, managed devices, copied text, downloaded files, and the device itself are outside this site's control. Use a device and network you trust. This page describes the active builder in this codebase; if data handling changes, the product copy and this page must change before release.
